Power of Attorney

Doctors and nurses of Annapolis Pediatrics are often asked for information on Medical Powers of Attorney. This legal document is important in that it allows someone you know and trust to make decisions for you concerning health care for your child in the event that you are not able. These forms may be necessary if you leave your children in someone else’s care while you (their legal guardian) are away for vacation or work travel out of the area.

Anne Arundel Medical Center provides a form for your use on their website. As members of AAMC’s medical staff, we refer our patients to AAMC on this matter. We are not legal advisors and recommend you consult an attorney or AAMC’s Patient Advocacy if you need additional information.
