Prescription Refills
For your convenience we can now have three ways that you can request a pharmacy refill:
- Call the office, select Option 5 and follow the prompts.
- Download the pharmacy refill form and fax to 410.263.7111.
- Download the pharmacy refill form and email to rx@annapolispediatrics.com.
It takes 72 hours to process your refill request. If your child is on a maintenance medication that requires regular medication recheck visits, please know that your medication may not be refilled if your child has not had a medication recheck in the past 6 months. If your medication request is denied for any reason a member of our staff will contact you. We will also leave a message on the phone number provided to let you know your request has been processed.
NOTE: Downloading the pharmacy refill form requires the free adobe acrobat reader program. Once you fill out the form, click the "save" button as shown below, and either print/fax or email to our office.
